Many people do not think about what it takes to live well with a disability or age-related concerns until they need help or a crisis occurs. You can prepare for the many transitions that occur in a lifetime by planning. Whether you are responding to a current need or planning for the future LifeLong Links can help you find your way through an array of choices, information, and options.

You can make decisions about the services you or your loved one need by using the information on this site. A broad range of services and support is available, even when a person’s independence is limited. In the aging community, these options are sometimes called “long-term care” or “aging in place.” In the disability community, these are “community supports” or “supports for independent living.” This Web site provides links for both populations.
